I've tried the TVout offset and have a centred picture now on my TV - the settings I settled on in the end were:
<PVR350Width>640</PVR350Width>
<PVR350Height>520</PVR350Height>
<PVR350OffsetX>60</PVR350OffsetX>
<PVR350OffsetY>20</PVR350OffsetY>
I drew a 3 pixel yellow border around the background and fiddled with the settings until I could see all of the borders.
This is when my TV is in 4:3 mode - perhaps an enhancement would be to be able to set the offset dynamically depending on the format of your TV (mine does 4:3, 14:9, and 16:9) - you could use the "full" button on the remote to cycle through the appropriate sizes?
Also I tried changing the width and height to match the PAL output (720x576) but this caused the picture to remain in the same place all the time even when I set an offset of 400!
