Update: Didn't seem to work. I must have not been properly paying attention last night because I turned it on this morning and it immediately presented with the same issue.

NEW UPDATE: I've tried all Hardware Decoders... I even tried turning it off. Still the same issues persist.
Any help is appreciated. I can provide new logs, whatever you need. Daz

GIGO. Turn on verbose logging (hold Ctrl on the main setting page to see it. Play live tv until the problem occurs and send the logs.

I think I can see what is happening (they are coming from multiple servers) and when one of the servers send a payload which was earlier then any that had been sent, it was inserted rather than skipped. Sub would need to confirm.
It seems to be missing #EXT-X-MEDIA-SEQUENCE too but I don't know what that means. I would delete that second one two. I have both and will give it to sub. Martin

Is there a difference between the way NPVR and other IPTV players handle this?
I'm still trying to understand why it is isolated to NPVR.

To me it seems like a bug in NextPVR's handling of this scenario. it doesn't really explain the problem in the other file since I would think this only happens at the beginning unless the servers are not really load-balancing and their streams aren't exactly the same.
Do you have an option on ts files instead of m3u8? Martin

I don't unfortunately. My provider only gives m3u8
