Scrambled picture probably due to bad tuning or decoder settings

Help!

I've spent over an hour now trying to figure out why my picture looks like the scrambled pic attached.

I'm running a PVR250 in the USA. I've typed numbers from the tuning helper into the EPG, tried both Antenna and Cable settings, different decoders, and if anything I only make things worse.

The WinTV2000 application from hauppauge runs perfectly so I'm sure I've made some boned headed setting somewhere or another.
