There are some quirks when using the HD-PVR for live tv. I noticed the same behavior as you with the .mpg extensions as well as additional problems because I use timeshift. I was not able to get the HD-PVR to work correctly with timeshift which is a must because I have several client when using the built in Analog HighDef Recording plugin. I setup it up using the graphrecorder plugin and it has been working great for a couple of weeks.
I do not have any playback problems with dropped frames as you described. I did have some issues with the Arcsoft decoders but have since switched to Cyberlink decoders using EVR on Vista and an ATI HD4550 so that I get hardware assisted decoding. This combination provides great playback for me with 1080 content on both timeshift and recorded material from the HD-PVR.
