Anyway, I tried your suggestion to set <ForcedGraphTeardown> to true. This seems to have solved the problem for me. Out of curiosity, what does this setting do? and why did the new version of HDHomerun software affect it?
With older HDHR software, if you were streaming one instance and started a second instance, the second instance would take control of the tuner. I suspect that with the code changes for the new locking features, the second instance can no longer assume control of the HDHR. So if the first filter graph was not completely disposed (when you press ESC), the second instance (when you start LiveTV again) does not get control and does not receive the stream.
This is all conjecture of course since I wasn't able to actually reproduce the problem.
