Unfortunately it's not clear to me why it's failing. Bascially it's just getting an very non-descript error trying to downoad the m3u8 and/or segment files.

That is odd...I've used this IPTV service provider's URLs in other IPTV apps and they play these URLs just fine. Minus the ability to record live tv. I'm wondering if I'm doing something wrong during the NextPVR install/setup. Any suggestions...like should I insert the EPG URL in the device settings? Or, should I just skip that and add the EPG in the Guide > EPG Sources? If I insert the EPG info into both these locations does that possibly create a problem/conflict? Any other tips...? Thank you!

You can turn on verbose logging (Settings - Ctrl - Enable Verbose) which provides more information about the m3u8 download process. The logs might contain sensitive data so only upload them after editing them or adding a password to the zip which you can PM to sub. I will look too if you PM me.
Also make absolutely sure you aren't using IPTV in any other app will NextPVR is running just in case it is a provider limit. Martin

In either case, do you want me to try and just play a program and send logs? Just record a program only and send logs...etc? Any suggestions on whether I should: 1) load the EPG URL in the Devices Setting, or 2) add the EPG in Guide EPG sources pointed to C:\EPG\guide.xml, or 3) both, or 4) only one location or the other? Can I add C:\EPG\guide.xml in the devices settings or can this location only manage/understand an EPG URL? I do not have this IPTV service, or any other IPTV service, running on another app while trying to configure NextPVR.

I don't see any reason to reinstall NextPVR. You have two distinct problems that need to be handled separately. The most import one is m3u8 downloads failing. There is no point in getting the guide until you can watch live. You should work with your existing setup and enable verbose logging and see what happens first.

Any other tips...?Usually you'd with IPTV you'd only ever add an EPG URL when you're initially using the 'Import M3U' feature, and it asks for the m3u and xml urls. (this is only about EPG though, and will make no difference to your ability to have stream work for longer) Reinstalling is unlikely to make any difference.

The 5 Star Max East program played for 3:24 minutes. When the video playback stopped, I pressed the Pause/Play button, and when the program resumed it played the exact same video scene for 3:24 minutes. I did this multiple times until the timer shown in the bottom left, of the Web App Video screen, reached 20 minutes. At this point, I pressed the Pause/Play button and, once again, the same 3:24 minutes of video played then stopped. I also tried playing a program from the A&E (East) US. That A&E (East) US program only played about 11 seconds. Pressing Pause/Play button resulted in the same 11 seconds of video to play again - as previously described in this post. ALSO, when I selected the A&E (East) US from the TV Guide screen, the program plays for 11 seconds. WHEREAS, if I select the same A&E (East) US program from the Channels screen the program will not play at all and returns this error message Streaming Failed (transcoder exited). I'll PM you and Sub with the verbose logging files. Thank you for your help!

Here's another example of the video playback from a randomly selected program from the Web App's TV Guide. As you'll see, the video contains THREE identical video segments that are each about 20 seconds long.
