When sharing your NMT folder, did you grant users change permissions? That did the trick for me.
GBPVR v1.4.7
Windows 7 Ultimate (64-bit)
Intel Core 2 Duo 2.33 GHz
4 GB RAM, 160GB system drive
640GB recording drive
PVR-500 - analog cable stations
HDHomeRun - ASTC via antenna
nVidia GeForce 8600GT
1 PCH @ 1080p componenent (was NTSC via composite)
1 PCH @ 1080p HDMI (was component)

This is not a requirement of mvpmcx2 or the scripts. What sometimes happens is people drag and drop files into a share which means these files won't have the correct permissions. Afterwards if permissions on the share change, all files permissions will change too.

Makes sense - that's how I copied them to the share...

I'm able to play video from the share(!)

I must be doing something wrong when I grant users change permissions on the folder...

Compress and upload your telnet.html and script.cgi file and I will have a look.

Ok, I don't see anything special. I would try one at a time.

1 - change @NMT to @nmt in telnet.html
2 - change ,rw to ,ro in script.cgi

This should be easier to debug with the next firmware.

Unfortunately I'm having the same problem. But since other user can make this work, there must be something that I have forgot to do...

Thank you for trying to help me though!

Could you temporary replace your script.cgi with this

Code:
#!/bin/sh

echo "Content-Length: 70";
echo "Content-type: text/html";
echo "";

echo "<head><body><H3>";
echo | ls /opt/sybhttpd/localhost.drives/ ;
echo "<BR/>";
echo "</H3></body></head>";
echo ".................................................................................";
exit 0;
