I'm not using any of the updated files from the survival guide (PVRX2.exe and NativeUtilities.dll). I am using the ATI MPEG Mux and not TS Mux, too many issues with TS Mux for me. Also, I pretty much left config.xml with the defaults in the BDA section except for the following:
<BDASubmitTuningRequestTwiceOnLivePreview>true</BDASubmitTuningRequestTwiceOnLivePreview>
I did add the following line to my bda.ini file under the [Hauppauge HVR-2250 (QAM)] entry to avoid analog/digital recording collisions on the same tuner:
SHARED_NAME=Hauppauge HVR2250
The qam.ini file I'm using when I scan is the original file that came with 1.3.7 that has the 138 channel numbers.
If I remember anything else that I have done to get this card working or things I noticed during installation, I'll let you know. Let me know if there are any other specific settings you would like to know.
