If you run the config app, go to Plugins tab, Add a folder, and move a single item into the folder, then run pvrx2 and select the folder, when you have the Back item selected it's in the middle as expected, but the single item isn't.

Sorry, there is known issues with having less than five items in submenus. You'll just try to work around it for now.

I'd like to add that the back button on my mce-remote does not work in these folders. And when 'Back' is selected (on screen) it does not go back right away. First it seems to rewrite the the screen and then it goes back one step.

I second that, Back button doesn't go back on menus.

Just try to stick to 5 or more sub menu items for now, or leave the items in the main menu.

It is better than 1.0.08 though. In 1.0.08 it was locking up for less than 5 items. In 1.0.16 it only seems to have a problem with 1 item in a submenu. Submenus with one item don't really make sense so except for the back button, submenus are working pretty well now for me.
